Here at OSUIT we use the Guardian Conduct system to handle Incident Reporting online. The following are the different types of incidents that can be reported and what they mean.
General Incident Report - Students and Employees may use this form to report any incident they believe is a violation of University policy and/or a law.
Title IX Incident Report - Students and Employees may use this form to report sexual assaults and harassment issues. From more information visit https://osuit.edu/1-is-2-many/assault.php
Person of Concern - This form can be used to report an issue where you are worried about someone on campus. This is used for when you feel like someone needs help or assistance but are unsure of what to do. Submit this form and someone will review the submission to determine how the University can assist the person. This report is not intend to report items for Student Conduct but for informational purposes.
